Job summary

McKesson is seeking a Field Nurse Case Manager to support patients in Ontario within a Patient Support Program. You will provide education, clinical support, case management, and therapy coordination while collaborating with healthcare professionals, pharmacies, and reimbursement teams.

The role combines patient care, care coordination, and navigation to improve outcomes, access therapy, and deliver an exceptional patient experience. Hybrid field-based with travel and home-office time.

Qualifications

• RN/RPN/LPN/LVN license in Ontario in good standing.
• Direct patient care and education experience.
• Ability to manage multiple priorities in a fast-paced healthcare setting.

Responsibilities

• Deliver patient-centric case management within program protocols.
• Support onboarding, education, adherence, and follow-up for patients.
• Provide disease-state, therapy, and medication education to patients and providers.
• Coordinate care with physicians, clinics, pharmacies, and internal teams.
• Collaborate with Reimbursement Specialists on funding and prior authorizations.
• Document interactions and maintain complete records in program systems.
