Job summary

McKesson is seeking a bilingual Field Nurse Case Manager (RN) to support patients and clinicians in a Patient Support Program across the Greater Toronto Area. You’ll be the key clinical resource, helping patients access therapy and stay engaged in their care journey.

This field role requires regular travel (about 60% of the time) with some remote work from a home office. Ontario RN license and PSP experience are essential.

Qualifications

• Active and unrestricted RN license in Ontario.
• Fluency in French and English (written and spoken).
• Experience in Patient Support Programs (PSP).
• Valid Ontario driver’s license and access to a reliable vehicle.
• Ability to travel up to 60% across the GTA.
• Strong patient education and communication skills.
• Proficiency with Microsoft Office and digital documentation systems.

Responsibilities

• Act as the primary clinical contact for patients, physicians, and clinic staff in the PSP.
• Support patient onboarding and enable timely access to prescribed specialty medications.
• Deliver patient education on diseases, treatment expectations, medication administration, and adherence.
• Provide injection training in clinic, remotely, or at home as required.
• Coordinate care with healthcare providers and communicate updates.
• Assist with reimbursement and patient access activities.
