Job summary

The Jean Tweed Centre seeks a Nurse Practitioner to provide non-urgent primary care assessments in person or via telemedicine, supporting continuity and transitions for clients and families. The role emphasizes collaborative leadership, formal education, and coaching to improve quality of care.

The position is remote, offering up to 30 hours per week, mainly daytime hours, with four days of coverage monthly for admission cycles at residential houses.

Qualifications

• Bachelor’s degree with advanced preparation in primary care or adult nursing.
• Basic Cardiac Life Support (BCLS) certification.
• Member in good standing with the College of Nurses of Ontario with advanced RN Extended Class certification.

Responsibilities

• Provide direct clinical assessments and primary care within RN extended class scope.
• Use telemedicine to relate to clients at remote locations and determine care needs.
• Educate patients individually or in group sessions on health topics.
• Collaborate with interprofessional team for care planning and follow-up.
• Monitor telemedicine services and report issues and gaps.
