Job summary
Fonemed is expanding its telehealth services and seeks passionate Nurse Practitioners to join a growing team in a work-from-home setting. You will assess patients via telephone and video, and build care plans with flexible scheduling that suits your life.
As an advanced practitioner, you will diagnose, prescribe, order tests, and provide follow-up care within the continuum of care, while collaborating with other professionals and following provincial regulations.
Qualifications
- Graduation from a Nurse Practitioner program.
- Current licensure or willingness to obtain licensure as a Nurse Practitioner.
- Graduation from an approved School of Nursing.
- FAA certification in Family All Ages (FAA) is required.
- Strong clinical assessment and decision-making abilities.
- Demonstrated ability to work independently and in a collaborative team environment.
Responsibilities
- Assess patients, diagnose, prescribe, and plan follow-up in a virtual clinic.
- Coordinate treatment plans and referrals with other health professionals.
- Communicate findings and prognosis with clients and care teams.
- Advocate for clients and ensure continuum of care.
- Follow best practice guidelines.
