Job summary

Nexim Healthcare is seeking a Nurse Practitioner to provide virtual care to clients in Ontario. You will conduct secure virtual consultations, assess health histories, review symptoms, and manage care within your scope, collaborating with a multidisciplinary team to ensure continuity of care.

You will prescribe medications, develop treatment plans, educate patients, and escalate to in-person care when needed.

Qualifications

• Must hold CNO NP registration.
• Strong assessment and clinical decision-making skills.
• Ability to practise independently while collaborating with a multidisciplinary team.
• Comfort using virtual care platforms and electronic medical records.
• Access to reliable internet and private, confidential workspace.

Responsibilities

• Conduct virtual patient assessments, including health histories, symptom reviews and clinical evaluations.
• Diagnose and manage conditions appropriate for virtual care and the NP’s scope of practice.
• Prescribe medications and order or review investigations as clinically appropriate.
• Develop treatment plans and provide patient education, follow-up and preventive care.
• Recognize when patients require in-person assessment, urgent care or emergency services, and arrange escalation.
• Maintain accurate, timely documentation in the electronic medical record.
• Coordinate referrals and communicate with other healthcare providers.
• Follow professional standards, privacy requirements and organizational policies.
