Responsibilities

• Engage with patients via phone and email with professionalism and empathy.
• Process intake information, verify referrals, and determine service needs.
• Enter data accurately into the client management system and maintain records.
• Coordinate with internal departments and external partners to initiate services.
• Ensure intake procedures comply with privacy policies and standards.
• Identify and escalate barriers to service in a timely manner.
