Job summary

Cogir Senior Living is seeking Registered Practical Nurse professionals in Oakville, Ontario. You will deliver planned, patient-centered nursing care and work within a multidisciplinary team to support residents of retirement communities.

You will assess, document and update care plans while mentoring unregulated care providers. Competitive hourly wages reflect experience and provincial requirements.

Qualifications

• Registered Practical Nurse in Ontario with current license.
• Clinical knowledge for senior care and gerontology preferred.
• Ability to work well in a multi-disciplinary team.
• Strong communication with residents, families and staff.
• Proficient time management and documentation skills.

Responsibilities

• Provide planned, patient-oriented nursing care per provincial standards.
• Collaborate with the multidisciplinary team and maintain clear comms.
• Implement and monitor care plans, adjusting as needed.
• Complete clinical assessments, care plans and documentation accurately.
• Coach and mentor unregulated care providers.

How YOU make a difference

• RPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
• RPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.
• RPNs implement and evaluate the effectiveness of the care plans and make changes as required.
• RPNs complete and ensure resident’s clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.
• RPNs teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.

What YOU bring

• Current registration as a Registered Practical Nurse in the applicable province
• Demonstrated sound clinical knowledge
• Experience working with seniors and with unregulated care providers preferred
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
• Strong drive, respect and an understanding of the emotional and physical demands of the role
• Focus on teamwork and customer service with the ability to build trusted relationships
• Strong written and oral communication skills and critical thinking skills
• Demonstrated organizational and time management skills
• Computer proficiency and experience with Microsoft office applications
• Current Basic Cardiac Life Support and First Aid certification required
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
