Job summary

Cogir Senior Living in Ontario is seeking a Registered Practical Nurse to join our team and provide high-quality, patient-centered care to residents. You will work within a multidisciplinary team, implementing and evaluating care plans while maintaining accurate documentation.

We value compassion, teamwork and strong communication. RPNs must be registered in Ontario with current CPR/First Aid and a willingness to mentor unregulated care providers.

Qualifications

• Registered Practical Nurse (RPN) with Ontario registration and license.
• Strong communication and interpersonal skills for resident and family interactions.
• Ability to work within a multi-disciplinary team and follow care plans.
• Proficient with documentation and basic computer skills.

Responsibilities

• Provide planned, patient-centered nursing care per provincial standards.
• Collaborate with a multi-disciplinary team and communicate with residents, families and staff.
• Implement and evaluate care plans; update resident documentation accordingly.
• Teach, delegate and mentor unregulated care providers as required.

How YOU make a difference

RPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

RPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.

RPNs implement and evaluate the effectiveness of the care plans and make changes as required.

RPNs complete and ensure resident’s clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.

RPNs teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.

What YOU bring

• Current registration as a Registered Practical Nurse in the applicable province
• Demonstrated sound clinical knowledge
• Experience working with seniors and with unregulated care providers preferred
• Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
• Strong drive, respect and an understanding of the emotional and physical demands of the role
• Focus on teamwork and customer service with the ability to build trusted relationships
• Strong written and oral communication skills and critical thinking skills
• Demonstrated organizational and time management skills
• Computer proficiency and experience with Microsoft office applications
• Current Basic Cardiac Life Support and First Aid certification required
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
