Job summary

SE Health in Ontario is hiring part-time nurses for community and school-based shifts, including day, evening and night hours. Opportunities span Central West and North York regions with travel between clients and school routes.

Competitive benefits and paid training accompany a strong supporting team focused on full-scope practice. Current ON, Canada nursing certification required, with immunization and background checks.

Qualifications

• Must hold valid Certificate of Competence with the College of Nurses of Ontario.
• Availability to work days, evenings, nights and every other weekend.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications.
• A current CPR certificate is required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
