Job summary

SE Health in Ontario seeks Part-Time nurses for community and school shift roles in Central West and North York Region. The role requires flexibility for days, evenings and night shifts, and travel between sites.

A $1500 sign-on bonus and competitive benefits are provided. You will join a supportive team with ongoing training, pension and education bursaries, and opportunities to grow within SE Health, a not-for-profit social enterprise.

Qualifications

  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Availability to work days, evenings, nights and every other weekend.
  • Updated immunization records and clear criminal background check are required on hire.
  • A willingness to attain SE Health certifications.
  • A current CPR certificate is required.

Responsibilities

  • Complete comprehensive client assessments and develop care plans.
  • When designated as a primary provider for a client caseload, be the point of care lead and coordinate with the team.
  • Support client transitions by connecting with community support services.

Responsibilities:

  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
  • When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Availability to work days, evenings, nights and every other weekend.
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required SE Health certifications
  • A current CPR certificate is required.
  • This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
