Temporary Part-Time 0.8 FTE – Maternity Leave

The Leduc Beaumont Devon Primary Care Network is a joint venture between participating local family physicians and Alberta Health Services. Our mandate is to sustain and enhance the quality, coordination, and integration of healthcare services in Leduc and area and to improve the quality of life for the community of patients and physicians.

The Leduc Beaumont Devon Primary Care Network seeks an engaged and enthusiastic Registered Nurse to join our multi-disciplinary healthcare team. The incumbent will deliver a broad range of nursing skills and services to a diverse population within physician clinics across the region as well as the PCN central clinic. Our teams are essential to the provision of community-based care (i.e. outside of AHS facilities), facilitating wellness, navigating the health system, and offering education and guidance.

Duties and Responsibilities:

  • Provide clinical care to PCN patients within the RN’s full scope of primary care practice;
  • Provide education, support, and lifestyle counselling for patients who are managing various health concerns;
  • Work collaboratively with a multi-disciplinary healthcare team;
  • Communicate with PCN member physicians on patient care planning and progress;
  • Facilitate health promotion classes virtually and in person;
  • Participate in quality improvement initiatives, standards of practice working groups and PCN committees.

Requirements:

  • A degree in nursing is required with current registration in CARNA;
  • Minimum 2 years of clinical experience in chronic disease and primary care nursing, preferably in a family practice or community setting;
  • Diabetes Educator Certification or willingness to obtain;
  • Demonstrated ability and willingness to work in an unstructured and dynamic environment that requires flexibility and collaboration with other health professionals;
  • Must have excellent written and verbal communication skills. Attention to detail is essential;
  • Effective time management and organizational skills;
  • Case management experience with large patient panels is desired;
  • Current criminal record check;
  • Must have own vehicle and valid driver’s license.

Salary and Hours of Work:

  • The salary range for this position is $39.34 – $50.20 /hour.
  • Working hours will be scheduled Monday to Friday from 8:00 AM to 4:00 PM. Late afternoon or evening hours may be required to teach classes.
