The successful candidate will provide total care for patients with diverse medical needs including dialysis and palliative care. Qualifications

• Experience in patient care including dialysis, dementia, palliation, and onco-care.
• Strong physical assessment skills are required.
• Ability to communicate effectively with patients and their families.

Responsibilities

• Responsible for total care of community patients in a complex medical/transitional care unit.
• Support the Philosophy of Patient-Focused Care.
• Work closely with the multidisciplinary team to deliver patient-centered care.
