St. Joseph’s Health System (SJHS) is seeking a Registered Nurse to support discharge planning for newborns in the Women’s Health and Newborn Care Program at the Charlton Campus in Hamilton. The RN will work with families to ensure access to information and assessment within 24–48 hours of discharge.
The role involves newborn assessments, bloodwork follow-up, and coordination with physicians, nurse practitioners, and public health services to support new moms and babies.

The Registered Nurse working at full scope of practice in partnership with family doctor’s nurse practitioners Midwives Public Health and Pediatricians supporting discharge planning within the Women’s Health and Newborn Care Program.
Responsible for the day-to-day support of the program supporting new moms and babies ensuring that families have access to information and assessment within 24-48 hours from discharge from the hospital.
The role includes assessment planning teaching newborn assessments and bloodwork follow up of obstetric and newborn patients and connecting new moms with any support that she needs.
