The RN collaborates with the health professional team and assumes a leadership role in coordinating and delivering a full range of care and treatment to patients/clients, including those with complex needs, high acuity, and less predictable outcomes.The RN is responsible for conducting comprehensive clinical screening and assessment to determine patient appropriateness for Post-Acute placement. This role supports hospital-wide capacity strategies through the coordination and management of patient flow, optimization of bed utilization, and active participation in admission, transfer, discharge, and repatriation planning.In collaboration with physicians, therapy teams, discharge planners, unit staff, consulting services, and external partners, the RN facilitates timely and appropriate transitions of care.
