Job summary

The Ottawa Hospital is seeking a Registered Nurse to join the Transitional Care Unit (TCU) on our General Campus. You will care for medically complex patients transitioning from hospital to home or another setting, in a 34-bed unit with a strong emphasis on collaboration and patient-centered care.

The role offers days and nights shifts, including weekends, with a focus on guiding patients and families through the transition and discharge planning to minimize readmissions.

Qualifications

• Certificate of registration from the College of Nurses of Ontario.
• Minimum of three (3) months of general nursing experience.
• Theoretical knowledge of the nursing process and ability to apply it in a clinical setting.
• Understanding of geriatric care and ability to apply it
• Ability to assess learning needs and plan teaching accordingly.
• Ability to work effectively in a collaborative multidisciplinary approach to patient care.
• Ability to assess discharge needs and participate in planning.
• Ability to communicate effectively with patients, families, and health professionals.
• Commitment to personal continuing education and self-evaluation.
• Willingness to participate in the hospital's Quality Assurance Program.

Position Information

Join our team as a Registered Nurse on the Transitional Care Unit (TCU) located at the General Campus!

On this 34-bed unit, you will provide specialized care to our patient population of medically complex patients transitioning from the hospital to home, or to another care setting.

The Transitional Care Unit is a highly motivated and results-oriented interdisciplinary unit who consistently demonstrates an innovative, creative and collaborative approach to drive better outcomes for patients.

Nurses who work on the Transitional Care Unit will have the opportunity to develop specialized skills and knowledge that will serve them for the rest of their career.

Rotation/Shifts

Days and nights, including weekends, 12 hour shifts.

What You Will Do

The Registered Nurse (RN) prioritizes the relationship with both the patient and their family by providing high-quality, patient-centered care, along with coordinating care with a focus on the patients transition through the continuum of care. In this role as patient-advocate, the RN will provide guidance in the development of self-management skills, with the goal of ensuring a smooth and successful post-acute care discharge, and significantly lowering readmission rates.

Basic Requirements

• Certificate of registration from the College of Nurses of Ontario;
• Minimum of three (3) months of general nursing experience;
• Demonstrates theoretical knowledge of the nursing process as it relates to the general medicine patient and the ability to apply this knowledge in a clinical setting;
• Theoretical knowledge of the care of the geriatric patient, and ability to apply this knowledge in a clinical setting;
• Demonstrates the ability to assess the patient's learning needs and ability to plan appropriate teaching;
• Demonstrates the ability to work effectively in a collaborative multidisciplinary approach to patient care;
• Demonstrates the ability to assess the patient's discharge needs and participates in the planning process;
• Demonstrates the ability to communicate effectively in an oral and written format with patients, public, colleagues and other health discipline;
• Demonstrates interpersonal and problem solving skills;
• Identifies a commitment to personal continuing education activities;
• Demonstrates an understanding and a willingness to participate in self-evaluation;
• Demonstrates an understanding of the application and relevance of the Quality Assurance Program and participates in unit activities;
• Must be eligible to work in Canada.

Preferred Qualifications

• A degree in Nursing;
• Proficiency in English and French oral expression (level 4 - advanced) and comprehension (level 4 - advanced).

The Ottawa Hospital uses a 5-level language proficiency scale to define the required level of language skills in English and in French for positions at the Hospital, and to evaluate job candidate language skills. Language proficiency requirements are based on the nature of the position and the level of interaction with French and English speaking clients.
