Job summary

SE Health in Ontario is seeking a Registered Nurse to join our caring team across Palmerston and surrounding areas. This full-time role offers a sign-on bonus, comprehensive benefits, and opportunities to grow while delivering high-quality care.

You will complete client assessments, develop care plans, coordinate transitions, and work with a supportive team to meet each client's unique needs. A valid RN Certificate of Competence is required, with on-call components and weekend shifts.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario as RN.
• Availability to work FT days with every other weekend and on call component (Evening and Overnight 6 nights/month).
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario as RN.

• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
• Required to travel to Mount Forest/Palmerston/Drayton/Arthur/Wellington and surrounding areas.
