Our Village at University Gates, located in Waterloo, Ontario has an exciting opportunity for a REGISTERED DIETITIAN to join their LONG-TERM CARE village team.

To be successful as our new Registered Dietitian you will:
- Complete Resident Nutrition Risk Assessments for all new admissions
- Monitor and follow residents who are identified at high nutritional risk
- Determine person responsible for monitoring residents at moderate and low nutritional risk
- Recommend specific nutritional interventions and provide information to the Care Team about implementing recommendations, monitoring and follow-up as required
- Collaborate with the resident’s attending physician regarding the nutritional plan of care and associated assessment needs, i.e.: lab results, vitamin supplementation, diet changes, etc.
- Participate as a member of the Interdisciplinary Team in the development and evaluation of nutrition care plans which includes an assessment, resident centred goals and nutritional care plan to meet the individual needs of the resident and evaluate the outcome of the nutrition care interventions
- Review all resident weights on a monthly basis with the Care Team and initiate appropriate interventions and documentation as necessary
- Reviews and/or update resident diet cards, nourishment lists, diet census sheet and dining seating plans as requested by the Director of Food Services
- Possess experience in clinical nutrition and geriatrics
- Knowledge of RAI-MDS and LTC experience is essential
- Have an Undergraduate Degree in Food and Nutrition, or equivalent, from an approved University.
- Successfully completed an accredited Dietetic Internship
- Current registration with the College of Dietitians and Dietitians of Canada
