The Norfolk Family Health Team (NFHT) is a community-based not-for-profit organization dedicated to providing quality, person-centered health care for individuals and families residing in Norfolk County.
Norfolk FHT provides access to family physicians, nurse practitioners, midwives, nurses, social workers, dietitians, pharmacists, and other health professionals. Norfolk FHT provides specialized services for rostered patients, including the Diabetes Management Program, Geriatric Program, Registered Dietitian, Mental Health Counseling, Cancer Screening, Well Baby and Childhood Immunization, Spirometry Clinic, and Pharmacy Medication Reconciliation. Community Programs include the Chronic Non-Cancer Pain Management Program, Norfolk Prenatal and Newborn Health Program, Sexual Health Program, Seasonal Agriculture Workers, Foot Care, and Smoking Cessation.

Job Description
As an integral member of the interdisciplinary team, the Registered Dietitian provides high-quality medical nutrition therapy in collaboration with patients, families, and care providers. The position includes individual counseling and group education for rostered patients and participants of the Chronic Non-Cancer Pain Management Program and the Norfolk Prenatal and Newborn Health Program. The dietitian provides nutrition care across the lifespan in a variety of areas, including infant, prenatal and postpartum nutrition, food allergies and intolerances, nutrient deficiencies, emotional eating, eating disorders (medically stable), chronic disease management, and health promotion. The Norfolk FHT RD program is in good standing with dietetic internship programs, whereby there is an opportunity for dietetic preceptorship. Given the goal of expanding the dietitian role at Norfolk FHT, there is also a strong focus on innovation, program planning, and evaluation in order to optimize nutrition care for Norfolk County.
