Skilled and Caring Registered Dietitians are who we need for our Community work!

Location: Courtice, Bowmanville, Clarington, Newcastle

Schedule:5 visits/week (potential for more)

As Durham Region’s largest community based rehabilitative therapy provider we have been providing community rehabilitation services for adults and children since 1997. Our Vision is to be recognized as a trusted and quality provider of healthcare services, helping adults and children to maximize their functional abilities within their environment and enable them to participate more fully in life’s activities and rewards. At Community Advantage Rehabilitation, we work together with our clients to advance their quality of life using best practices, innovation and quality standards.

We are currently seeking qualified Registered Dietitians to provide services in the community.

RD Responsibilities:

· Provide ministry funded, community based Registered Dietitian services within the client’s place of residence

· Provide Dietitian assessment (through interview, standardized and informal assessment techniques) in the areas of

· Develop and implement best practice and evidence-based intervention in collaboration with clients/caregivers

· Provide education to client/caregivers and link to community agencies/resources when appropriate

· Complete and maintain documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and college standards

Requirements:

· Registered Dietitian with the College of Dietitians of Ontario (required)

· Experience in community setting is preferred but will consider new graduates

· Pediatric experience and/or a willingness to learn how to address the needs of the pediatric population

· Ability to work independently and as part of an inter-professional team

· Experience in assessment and counselling

· Knowledge of chronic self-management principles

· Ability to prioritize, manage self-scheduling and meet reporting deadlines

· Excellent oral, written skills and report writing

· Proficiency with computers

· Strong interpersonal and organizational skills

· A willingness to cover Courtice, Bowmanville, Clarington, Newcastle, Newtonville

· An ability to help with overflow in the other areas of Durham Region as required

· Reliable transportation is required for this position
