Job summary

ParaMed Home Health Care in Canada seeks a Registered Practical Nurse to perform scheduled community visits, delivering ethical and safe nursing care and coordinating with RNs for complex cases. You will work with patients of all ages to keep them comfortable in their homes, with autonomy, independence, and a balanced work-life.

The role requires travel to patient residences within a designated area and on-call coverage after hours.

Qualifications

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability and flexibility.
  • Willingness to travel to and from patient residences within a designated area.
  • Experience working with the vulnerable population.
  • Passion for making a difference in your community.
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs.

  • Execute and continuously monitor care plans and adjust as required.

  • Maintain and update professional patient records related to patient encounters and plans.
