Job summary

Yee Hong Centre For Geriatric Care in Mississauga is seeking a casual Program Assistant to support the Day Program. Under supervision, you will help plan activities and deliver high-quality services to clients.

Responsibilities include coordinating activities, ensuring safety and hygiene, providing crisis intervention, and supporting clients, caregivers, and volunteers. The role also involves record-keeping, care plan collaboration, and cross-site availability.

Qualifications

• Diploma in Social Work or Social Services, health sciences, activation, or equivalent.
• Experience in geriatric rehabilitation is essential.
• Excellent customer service skills.
• Ability to work independently and contribute effectively as a team player.
• Computer literacy in Microsoft Office Suite.
• Recent CPR certification.
• Recent satisfactory Police Vulnerable Sector Screen result.
• Fluency in English and ability to converse in Chinese.
• Must be able to work across different Yee Hong sites as needed.

Responsibilities

• Under the overall direction of the Senior Coordinator/Program Coordinator, participates in the planning of appropriate activities for clients of the program.
• Implements and ensures smooth operation of the activities in a safe, hygienic and supportive environment.
• Provides timely and appropriate crisis intervention or behaviour management and ensures that the clients' physical, social and emotional needs are appropriately met.
• Provides timely and appropriate support/information and referral/placement/outreach to clients/volunteers (as appropriate) to ensure that the clients/volunteers' social/ recreational/development needs are appropriately met.
• Works closely with client and caregiver in ensuring appropriate design of care plan/program plan and ongoing monitoring of plan.
• Ensures appropriate assessment, placement, matching and support for volunteers.
• Provides effective guidance, training and support to the volunteers in the program.
• Maintains up to date and accurate files, records and statistics on the program.
• Maintains accurate record of fees and expenditures.
• Complies with risk management policy for health and safety and infection control.
• Keeps up-to-date with developments and best practices in the field.
• Other duties as assigned.
