Job summary

Yee Hong Centre for Geriatric Care in Toronto, Ontario, seeks a Part-Time Program Coordinator for Home Support Services to oversee daily coordination, staff scheduling, and client support in the community. You will coordinate meals and transportation, monitor health, and maintain records and care plans with attention to compliance and quality of care.

The ideal candidate holds a Bachelor’s in social services or related field, 2+ years of experience, CPR, and a bilingual English/Chinese skill

Qualifications

• Bachelor's degree in social services, health care, activation, or equivalent.
• Minimum 2 years experience in social services or health care; supervisory experience is an asset.
• Knowledge and experience working with seniors; CPR certificate required; police reference check with vulnerable sector screen.

Responsibilities

• Coordinates delivery of home support services to enable clients to age in place.
• Coordinates meal and transportation services for clients.
• Maintains up-to-date health records and policies for the program.
• Supervises and trains staff/volunteers; manages resources and schedules.
• Ensures risk management and a safe environment for clients and staff.

Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.Program Coordinator, Home Support Services (Part-Time)The incumbent is responsible to the Program Manager/Assistant Director of Home Support Services, for the overall day to day coordination of Support Services to the communityResponsibilities:Coordinates the delivery of appropriate home support services to enable clients to age in placeCoordinates meal and transportation services for clientsProvides timely and professional client intervention to clients and familyMonitors overall health status and keeps up-to-date health records of clientsCoaches, develops and evaluates staff to ensure they meet their required performance standard.
