Job summary

Garden River Wellness Centres in Garden River First Nation is seeking a Program Assistant for Home and Community Care Support Services. This role provides comprehensive administrative and operational support to ensure prompt, effective client service and fosters meaningful client relationships.

The ideal candidate has a diploma in medical/office administration, familiarity with PS Suites and Alaya Care, and at least two years of administrative experience in a health-care setting.

Qualifications

  • Knowledge of healthcare office management systems and protocols.

Responsibilities

Role Summary

We are currently looking for a Home and Community Care Program Support Services, Program Assistant, that will be responsible for providing a complete range of operational and administrative support for the assigned team in their daily activities to ensure that clients receive prompt, effective customer service. This role fosters meaningful relationships, supports client engagement, and strengthens holistic service delivery.

What You Will Do
  • Provide day-to-day administrative support to the Home and Community Care Support Services Program
  • Provide support around general office duties and administrative tasks required in the health care setting.
  • Receive and direct incoming telephone calls, emails, and inquiries from clients, families, caregivers, and outside community partners.
  • Schedule client visits according to approved care plans, authorized service levels, staff availability, and program priorities.
  • Receive and track new referrals from clients, families, physicians, hospitals, and community organizations.
  • Assist with administrative processing and coordination of approved services, medical supplies, equipment, and related resources.
  • Maintain referral, service, scheduling, equipment, and communication documentation.
  • Assist with NIHB, Jordan's Principle, Ontario Health atHome, HNSA, MOLTC, or other funding documentation when applicable to the client's services.
  • Process and assist in managing confidential patient records ensuring compliance with privacy policies and regulations
  • Maintain Life alert System data base
  • Maintain appropriate supply and equipment records for Home and Community Care Support Services Program.
  • Assist with maintaining program statistics and administrative data
Required Skills
  • Required Diploma in Medical/office administration, business administration or a related field is preferred.
  • Familiarity with Electronic Medical Records and scheduling; specifically, PS Suites and Alaya Care.
  • Proven Administrative experience of 2 years minimum
  • Knowledge of administrative operations using advanced knowledge of healthcare office management systems and protocols.
  • Ability to work flexible hours including evenings and weekends.
  • Occasional travel when required.
  • Valid G license and access to a reliable vehicle
  • Clear CPIC records check as a condition of employment
  • Knowledge, understanding and experience of First Nation culture, values, and traditions.
