Job summary

Sioux Lookout First Nations Health Authority is seeking an Aide - Community Assistant to support allied health services and administer the Community Health Hub. It is a community-based role headquartered in Nibinamik (Summer Beaver) with potential for placement in other communities based on client load.

Qualifications

  • First Nations ancestry preference as per posting.
  • Must live in one of the First Nation communities listed under location.
  • Partial high school level education is acceptable.
  • At least one year of experience with health or education providers.
  • Valid full G-class driving license is required.
  • Demonstrates integrity and ability to maintain confidentiality.
  • Excellent organizational and scheduling abilities are essential.
  • Familiarity with MS Word, Excel, Adobe Acrobat and internet-based tools; willingness to learn SLFNHA systems.
  • Willingness to share knowledge and culture of First Nations communities and to advocate for community health priorities.
  • Ability to work as a team member or independently with minimal supervision.

Responsibilities

  • Organize pick-up and drop-off of staff at the airport.
  • Coordinate bookings for the Community Health Hub and manage key access.
  • Clean and maintain the building per LOA with the community.
  • Ensure supplies are ordered and maintenance requests are processed.
  • Access and utilize WellSky and Mustimuhw systems for travel and records.
  • Support and carry out therapy plans for clients and monitor progress.
  • Document and report progress through appropriate channels.
  • Identify potential new clients and facilitate referrals.
  • Assist with clinic and home-visit coordination for visiting professionals.
  • Provide on-the-job training and service support as needed.
  • Perform other duties as assigned.

Job Summary: Under SLFNHA's Developmental Services, the SLFNHA Aide - Community Assistant will support allied health services delivered by SLFNHA and the administrative duties of the "Community Health Hub". This would include the maintenance and management of the building, arranging transportation to and from airport for staff, support booking of clients, providing support to individual clients and families by follow-up, monitoring and carrying out services as directed by SLFNHA Health Care Professional staff.

The SLFNHA Aide - Community Assistant will be accountable to the Manager – Clinical, Developmental Services and Community Coordinators for mentorship and day-to-day support.

The position will be Community based out of Nibinamik ( Summer Beaver ). Other First Nation communities will be considered based on client load. Full-time is preferred; part-time may be considered.

Salary Range: $51,319 - $68,932 per annum. $28.197 - $37.874 per hour.

Qualifications

  • Must live in one of the First Nation community listed under location
  • Equivalent to partial high school education
  • One year of experience working with health or education providers

  • Professional integrity and the ability to maintain confidentiality is essential
  • Excellent organizational and scheduling skills
  • Familiar with the use of popular software such as MS Word, Excel and Adobe Acrobat (PDF) and skilled at use of internet-based tools and willingness to learn SLFNHA scheduling and ordering systems
  • Willingness to share knowledge and culture of First Nations people and specifically, the respective community
  • Willingness to speak up on the health and mental health priorities of your community
  • A team player who is able to work independently with minimal supervision
  • Knowledge of Anishinaabe language is an asset
  • The ability to perform the requirements of the position on a regular basis.
Roles and Responsibilities
  • To organize pick up/drop off of staff at the airport

  • Ensure supplies are ordered through SLFNHA and maintenance requested through community
  • Access and utilize Wellsky travel coordination and Mustimuhw community health record systems
  • To support and carry out therapy plans for clients
  • To monitor progress and report appropriately through documentation or consultation
  • To support the identification of new clients and make referrals
  • To assist with the coordination of clinics and home visits for visiting professionals
  • On job training and service support will be provided
  • Other duties as assigned
