Job summary

Georgian Bay Family Health Team is seeking Physiotherapists to join our team in a collaborative primary care setting. You will conduct primary care assessments, educate patients, plan care, and provide clinical guidance for rostered patients with musculoskeletal, mobility, pain, and functional concerns.

You will support timely access to conservative care, contribute to chronic disease management, and help optimize outcomes within a team-based environment.

Qualifications

• Bachelor’s degree in health sciences and active registration with the College of Physiotherapists of Ontario.
• Current First Aid and CPR certifications are required.
• Valid Ontario driver’s license and access to a vehicle is necessary.

Responsibilities

• Provide comprehensive physiotherapy assessments for rostered patients with musculoskeletal, mobility, pain and functional concerns.
• Deliver front-line consultation, triage, and early intervention for related conditions.
• Collaborate with physicians and interdisciplinary team members to support coordinated patient care.
• Support chronic disease management initiatives where physiotherapy input improves function and quality of life.
• Document assessments, progress notes, and care plans in the EMR in a timely manner.
• Promote patient education on injury prevention, self-management, and healthy lifestyles.
