Job summary

The Hamilton Family Health Team is seeking a part-time Physiotherapist to join a multidisciplinary primary care team. You will conduct musculoskeletal, neurological and functional assessments, develop individualized treatment plans and provide hands-on therapy, exercise prescription and patient education.

You will collaborate with physicians, nurse practitioners and other allied health professionals, support chronic disease management and patient navigation of community services, and contribute

Qualifications

• Masters of Science in Physiotherapy (MSc) or equivalent.
• Registered and in good standing with the College of Physiotherapists of Ontario.
• Proficiency with EMR systems (Telus PS) and virtual care platforms (e.g. Microsoft Teams).

Responsibilities

• Conduct musculoskeletal, neurological and functional assessments.
• Develop and implement individualized treatment plans.
• Provide hands-on therapy, exercise prescription and patient education.
• Collaborate with interprofessional team to support chronic disease management and falls prevention.
• Maintain accurate documentation in the EMR and communicate notes to the primary care team.
