Job summary

Mission Services of Hamilton is seeking a PITCH Case Manager for a 6-month full-time contract within the Emergency Shelter Services team. You will provide homelessness prevention, diversion and early intervention support to individuals on probation or parole, helping them work toward permanent accommodations.

  • Experience in case management techniques, use of and connection to community resources.
  • Ability to deal with confidential information and possess excellent interpersonal and diplomacy skills.
  • Demonstrated ability to form helping relationships based on trust and empowerment.
  • Evidence as a highly motivated team player, with proven ability to take initiative and be self-directed.
  • Certification in non-violent crisis intervention and First Aid/CPR.

  • Completes third party verification by contacting various sources.
  • Responds to crisis situations to promote safety for service users.
  • Maintains thorough case documentation in automated systems and updates intake records after each contact.
  • Performs other duties to ensure housing outcomes and safety policies.

POSITION SUMMARY:

As part of Mission Services’ Emergency Shelter Services team, the PITCH case manager will provide homelessness prevention, diversion and early homelessness intervention support to individuals on probation or parole working to secure permanent accommodations. The PITCH Case Manager will engage regularly with their service users to provide the necessary support to achieve program and participant goals

KEY RESPONSIBILITIES:

  • Conducts assessments by phone and in person to determine the needs and resources of individuals, in order to develop a short-term service plan to address their specific needs.
  • Applies homelessness prevention and shelter diversion strategies where possible, to assist service users to sustain their current housing and/or be directed to applicable service providers.
  • Provide service to individuals being released from corrections, on probation, who are at risk of becoming homeless or experiencing homelessness
  • Collects and assesses information to determine service user’s eligibility for emergency shelter and other shelter and housing related services.
  • Coordinates referrals to supports including community agencies, City of Hamilton programs, and other levels of government to meet service user needs.
  • Utilizes the principles of case management when conducting assessments and referrals to address service user needs.
  • Advocate on behalf of service users for services and benefits with internal and external resources and supports, which may include landlords/housing providers, income maintenance programs, shelters, and other service providers.
  • Completes third party verification by contacting various sources, including community agencies, housing providers, health care providers, income maintenance systems, child welfare agencies, Public Trustees, and other service providers.
  • Responds to crisis situations involving service users by assessing the situation and implementing appropriate solution which promotes service user safety.
  • Maintains thorough case documentation in automated systems, including documenting all intake and case plan information, and entering statistics related to case histories. Updates and revises intake records with each service user contact, to ensure case information is current.
  • Other duties as required to ensure the continuation of service to residents/clients as it relates to achievement of individuals’ successful housing outcomes
  • Work safely, identify any health and safety concerns and report to management, and follow Mission Services workplace health and safety policies and procedures at all times

QUALIFICATIONS:

  • Post-secondary diploma or degree in social services or related field
  • Experience applying Housing First principles and housing clients in community.

  • One to two (1-2) years of experience working with men involved in the criminal justice system.
  • 1-2 years Experience working with homeless adult males experiencing mental health and addictions issues, providing non-judgemental supports to them.
  • Experience in case management techniques, use of and connection to community resources
  • Ability to deal with confidential information and possess excellent interpersonal and diplomacy skills.
  • Demonstrated ability to form helping relationships based on trust and empowerment
  • Evidence as a highly motivated team player, with proven ability to take initiative and be self-directed.
  • Certification in non-violent crisis intervention and First Aid/CPR.

HOURS OF WORK:

  • 37.5 hours per week, Monday to Friday
  • Ability to work a flexible schedule of day, afternoon, evening in order to support staffing and program needs within a 24/7 setting
