Job summary

The Hamilton Family Health Team is seeking a part-time Physiotherapist to join a multidisciplinary primary care team. You will conduct musculoskeletal, neurological and functional assessments, develop individualized treatment plans and provide hands-on therapy, exercise prescription and patient education.

Position Overview:

The physiotherapist (PT) plays an integral role in delivering primary care services within a multi-disciplinary team. Working in collaboration with a team of physicians, nurse practitioners and other allied health professionals, the physiotherapist provides comprehensive assessments, evidence-based treatments and patient education to improve function and quality of life for patients across the lifespan with a focus on individuals that do not have access to physiotherapy supports in the community.

The PT will diagnose within their scope of practice and establish and implement evidence-informed treatment plans that include integration with existing community services. A strong knowledge of community-based programs is required to facilitate clients’ access to external services to meet individual needs and manage caseload demands.

Roles and Responsibilities:

Primary Care Integration

Attachment Efforts:

• Support patients who do not have primary care to be referred to the attachment team.

Access Efforts:

• Be an ambassador to the groups and workshops offered by HFHT by being aware of current offerings, handing out promotional materials and promoting the groups and workshops to existing, new and future patients.
• Contribute to improved access and continuity of care across local healthcare services.

Quality & Performance Improvement

• Ensure continuous quality improvements and adherence to best practices or leading practices related to physiotherapy and primary healthcare services, particularly in relationship to priority populations.
• Ensure services and programs follow ministry guidelines, HFHT policies, legislation and professional standards
• In collaboration with the team, participate in program planning, development and evaluation of physiotherapy services for individuals and within groups and workshops, as appropriate.
• Monitor data, patient outcomes and satisfaction to inform service provision improvements.
• Foster a welcoming, respectful, and inclusive care environment.
• Address patient concerns with patient-centeredness and professionalism.
• Foster a culture of patient engagement and shared decision making

Resource Optimization

• Support the effective use of resources to meet service demands
• Identify gaps and help develop processes that improve efficiency and capacity

Leadership

• Model the HFHT’s values of collaboration, inclusivity, accessibility and excellence
• Support and collaborate with team members to maximize team effectiveness
• Champion equity, diversity and psychological safety in the workplace
• Along with your team, serve as a leader, advocate and ambassador of physiotherapy in primary care
• Support student placements and mentoring opportunities when possible.

Physiotherapy related Duties and Responsibilities:

• Conduct musculoskeletal, neurological, and functional assessments.
• Develop and implement individualized treatment plans.
• Provide hands‑on therapy, exercise prescription, education, and self‑management strategies including encouragement to attend HFHT groups such as GLAD, Just Walk or other relevant programs.
• Collaborate with other team members to support chronic disease management, falls prevention and other preventative strategies.
• Maintain accurate, timely documentation in the electronic medical record (EMR) and communicate consult notes to the primary care team as needed.
• Assist patients with navigation of healthcare and community support systems.
• Collaborate with interprofessional team members to support coordinated, patient-centered care.

Qualifications and Skills

Required:

• Masters of Science in Physiotherapy MSc (PT) or equivalent
• Registered and in good standing with the College of Physiotherapists of Ontario
• Up to date professional liability insurance, membership of Canadian Physiotherapy Association / Ontario Physiotherapy Association.
• Proficiency completing clinical assessments and evidence-based care plans for patients with complex conditions while working with them to develop self-management skills.
• Experience and competency supporting individuals with chronic pain, musculoskeletal injuries, and vestibular conditions
• Excels at building effective, professional relationships with patients, provide a supportive approach to care and guide patients to work towards independence.
• Demonstrated clinical reasoning and therapeutic skills
• Strong communication and documentation skills
• Proficiency with EMR systems (Telus PS) and virtual care platforms (i.e. EMRs, Microsoft Teams)
• Ability to work independently in a collaborative, interdisciplinary setting
• Knowledge of relevant clinical practices, legislation and guidelines.

Preferred:

• Primary care, community, and team-based care experience is preferred along with a supportive approach to patients
• Understanding of quality improvement processes as they relate to patient care and program evaluation.
• Strong knowledge of best practices and benchmarks for advancing integrated care.
• Experience or interest in supervising student learners from physiotherapy and other health care professions
• GLAD certified preferred

Summary

Classification: Non-Union

Employee Class: Part-Time

Schedule: Monday-Friday; 16hrs/week
