The Job Summary
The purpose of this position is to provide comprehensive physiotherapy assessments, treatment, and care for community clients. The role involves conducting initial assessments and regular reassessments, documenting clinical information, and developing personalized Physiotherapy care plans in collaboration with clients and their families. The incumbent monitors client progress and modifies treatments to achieve specific goals related to transfers, ambulation, strength, range of motion, balance, and functional mobility. Additionally, they actively participate in continuous quality improvement initiatives, including measuring outcomes, gathering satisfaction feedback, mentoring clinical physiotherapy students and supervising Physiotherapy assistants.
Key Accountabilities & Success Criteria
- Conduct Physiotherapy Assessments for all new Clients referred to Outpatient Physiotherapy clinic
- Complete timely and thorough clinical documentation.
- Create and oversee client centred Physiotherapy care plans, establish personalized treatment goals in collaboration with the client and resident
- Assign tasks to Physiotherapist Assistants and other support staff
- Implement, monitor, and modify treatment to achieve or maintain client functional mobility goals
- Participate in continuous quality improvement for the clinic and organization including measurable outcomes, satisfaction feedback, and quality improvement planning
- Participates in reporting of Outpatient program Ministry client care outcome statistics
- Participates in mentoring Physiotherapy students and PTA/OTA students in accordance with CPO standards.
- Contributes to a transparent culture of client and staff safety by adhering to and abiding by patient and staff safety policies and procedures set by SJHCG
Qualifications
- Registrant in good standing with the College of Physiotherapists of Ontario
- Knowledge/experience in evidence-based musculoskeletal, neurological, and cardiorespiratory practice, with a strong emphasis on improving functional outcomes
Skills & Abilities
- Registrant in good standing with the College of Physiotherapists of Ontario
- Knowledge/experience in evidence-based musculoskeletal, neurological, and cardiorespiratory practice, with a strong emphasis on improving functional outcomes
- Superior communication and clinical reasoning skills; flexible and adaptable to changing clinical demands and schedules
- Caring and compassionate in all interactions with clients and families
- Preferred experience in delivery of Physiotherapy Services for the geriatric population, stroke, musculoskeletal, Chronic Obstructive lung disease and lower extremity amputee conditions groups
- Excellent time management, organizational and prioritization skills as relates to patient care
- Ability to demonstrate a strong customer service philosophy
- Demonstrated to be self-directed, seeking direction appropriately
