PharMerica is seeking a highly capable and motivated individual to join our team. We are a non-retail pharmacy focused on providing exceptional customer service and meeting pharmacy needs for hospitals, rehabilitation facilities, long-term acute care centers, and other specialized care centers nationwide.

Position Details:

• Schedule: ON-SITE, Monday-Friday 9:00am-1:00pm, rotating weekends
• Pay Rate: $62/HR

Responsibilities:

• Evaluate physician medication orders for appropriateness, potential interactions, and route of administration; recommend therapy changes as needed
• Maintain patient/resident medical records, including history, lab data, and medications
• Order and evaluate laboratory tests for monitoring high-risk conditions
• Collaborate with healthcare team providing clinical and technical support
• Document communications with healthcare providers and patients
• Implement policies for safe drug procurement, storage, and disposal
• Maintain regulatory documentation and report medication errors
• Assist staff with medication procedures
• Perform other tasks as assigned

Qualifications:

• Bachelor’s Degree in Pharmacy; Pharm D preferred
• One year experience in long-term care
• Licenses/Certifications as required by law
• Detail-oriented, goal-oriented, team player, good communication skills
