University Health Network (UHN) in Toronto West Park Healthcare Centre is seeking a Pharmacist for inpatient clinical units on a permanent part-time basis. The role provides patient-focused care with cross-coverage to other areas, working 18.75 hours weekly on days and weekends.

Responsibilities include pharmacotherapy assessments, medication histories and reconciliation, drug information, and collaboration with the healthcare team to optimize therapy and safety.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy required.
• Certificate of competency from the Pharmacy Examination Board of Canada required.
• Active registration with the Ontario College of Pharmacists (Part A) required.
• Hospital pharmacy practice experience preferred.
• Clinical experience in rehab is an asset.
• Hospital pharmacy residency certificate an asset.
• Membership in professional societies an asset.
• Excellent verbal and written communication skills.
• Excellent interpersonal and organizational skills.

• Provide patient-focused care to improve outcomes, including pharmacotherapy assessments, histories and reconciliation.
• Support drug distribution, access, procurement and medication safety.
• Engage in education, professional development and training of staff and learners.
• Contribute to projects aligned with pharmacy priorities and organizational objectives.
• Provide drug information to healthcare teams to promote safe, effective therapy.
• Communicate effectively with patients and the care team.
• Perform other duties as required.

Job Description Union: Non-Union New or Replacement: New Site: West Park Department: Pharmacy Reports to: Pharmacy Manager Wage Range: $48.51 - $60.64 per hour Hours: 18.75 Hours per Week Shifts: Days and Weekends Status: Permanent Part-Time Closing Date: October 8, 2026 Job Summary The Pharmacist will be responsible for providing patient-focused care to the inpatient clinical units at West Park Healthcare Centre with cross-coverage to other areas as required. Services include Acute Brain Injury (ABI) Behavioural, Amputee Rehab, Complex Continuing Care (CCC), Geriatric Rehab and Geriatric Low Tolerance Long Duration, Long-Term Ventilation, Musculoskeletal, Respiratory Rehab and Respiratory CCC, Stroke and Neurological Rehab, and Tuberculosis Rehab.

• Provides patient-focused care to improve patient outcomes; this includes identifying, resolving, and preventing drug-related problems by conducting thorough pharmacotherapy assessments, performing medication histories and medication reconciliation, providing drug information to patients and healthcare teams, and other patient care activities
• Supports operational patient care activities including facilitating drug distribution and/or access, procurement, and drug coverage; promotes medication safety
• Participates in education activities including professional development and continuing education; mentorship and training of new pharmacy staff and learners
• Participates in development, review, implementation, and evaluation of projects and initiatives in line with the priorities and/or objectives of the pharmacy department and/or organization
• To provide drug information services to members of the healthcare team to promote the safe, effective, and cost-effective use of medication therapy in the patient care setting
• Effectively communicates with patients, colleagues, allied health professionals, and other members of the patient-care team
• Performs other duties consistent with the job classification, as required.

• Possess a Bachelor of Science degree in Pharmacy, Doctor of Pharmacy degree or equivalent required
