University Health Network is seeking an experienced Pharmacist to join Toronto Western Hospital's Pharmacy, Inpatient Unit. You will provide comprehensive pharmaceutical care for admitted patients and contribute to patient outcomes.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy is required.
• Certificate of competency from the Pharmacy Examination Board of Canada is required.
• Active registration with the Ontario College of Pharmacists (Part A) is required.
• Hospital pharmacy practice with patient-centered care experience is expected.

Responsibilities

• Conduct BPMH interviews and reconcile medications for admitted patients.
• Review doctors’ orders for safety and appropriateness.
• Identify and resolve drug therapy problems with the health team.
• Monitor effectiveness, toxicity and adherence to evidence-based guidelines.
• Follow-up on medication incidents and promote safe practices.
• Provide drug information and patient education.
• Assist with discharge reconciliation and transition of care.
• Provide supervision of pharmacy technicians when needed.
• Support teaching and research within the department.

Department: Pharmacy, Inpatient Unit (Medical Surgical Neuro Intensive Care Unit (with cross coverage to other area as required)

Reports to: Pharmacy Clinical Site Leader

Wage Range: $48.51 - $60.64 per hour.

Hours: 37.5 Hours per Week

Shifts: Days, Evenings, Overnights, Weekends

Status: Permanent Full Time

Position Summary

The Pharmacist provides comprehensive pharmacy services to patients admitted to Toronto Western Hospital, as a member of the patient care team.

Duties

• Conducting best possible medication history (BPMH) interviews, performing medication reconciliation, review all doctors’ medication orders for appropriateness
• Identify, resolve and prevent drug therapy problems with the patient and health care team
• Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines
• Follow-up on medication incidents and promote safe medication practices on the unit
• Respond to drug information questions
• Patient education
• Facilitate discharge reconciliation
• Provide direct and/or functional supervision of technician staff
• Facilitate and support clinical teaching and research activities within the department, while exemplifying UHN’s values of patient needs come first, safety, compassion, teamwork, integrity, and stewardship

Qualifications

• Possess a Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent required
• Certificate of competency from the Pharmacy Examination Board of Canada required
• Active registration with the Ontario College of Pharmacists (Part A) required
• Hospital pharmacy practice experience with patient-centered care experience is a requirement
• Excellent verbal and written communication skills
• Excellent customer service and interpersonal skills
• Excellent organizational and time management skills
• Hospital Pharmacy residency an asset
• Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset
