Qualifications

  • Lizenziert bei der Ontario College of Pharmacists (Teil A).
  • Bevorzugt: drei Jahre Erfahrung in der Krankenhausapotheke oder Abschluss einer Krankenhausresidenz.

Job description

Position Summary

The Pharmacy Department provides medication management services to the entire hospital, including drug distribution and clinical pharmacy support. Pharmacists work within all clinical care teams, such as Critical Care, Medicine, Surgery, and Oncology. The department is committed to patient care, safety, and continuous learning.

Role Responsibilities

The Pharmacist collaborates with multidisciplinary teams to manage medications, aiming for optimal patient outcomes and safety. Responsibilities include monitoring medication issues, recommending therapies, improving quality of life, reducing hospital stay, and minimizing adverse effects and interactions. Pharmacists also participate in clinical services, interdisciplinary committees, and mentor students.

Qualifications, Experience, Skills, and Abilities
  • Licensed with the Ontario College of Pharmacists (Part A)
  • Bachelor of Science or PharmD in Pharmacy
  • Preferred: three years hospital pharmacy experience and/or completion of a Hospital Residency
  • Experience with Meditech and medication order scanning preferred
  • Intermediate proficiency in Microsoft Office and database management
  • Excellent communication, interpersonal, and organizational skills
  • Strong leadership, critical thinking, and autonomous decision-making skills
  • Ability to work independently and collaboratively in a fast-paced environment
  • Commitment to excellent customer service with patients, families, colleagues, and physicians
