Job description

**Job Description:**

This position demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, and cost-effective pharmaceutical care within the scope of their assigned practice. It is responsible for patient care and developing effective, professional relationships within the multidisciplinary team.

**This position works normal clinical hours, Monday - Friday. It is fully remote. Applicants must** **be licensed in CO or MT to qualify.**

**Essential Functions:**

  1. Reviews patients' medication orders for allergy interactions, duplication of therapy, appropriateness, and drug interactions. Makes evidence-based interventions to optimize medication use, ensuring therapies are safe, appropriate, and cost-effective, and medications are accurately dispensed.
  2. Oversees and participates in ensuring medications are prepared, delivered timely, stored securely, and appropriate for stability and workflow.
  3. Provides counseling to patients, promotes medication education and adherence, and ensures respectful, professional service.
  4. Completes regulatory and compliance training, maintains clear documentation, and adheres to laws, regulations, policies, and procedures.
  5. Supervises and directs activities of interns, technicians, and support staff. Provides clinical consultation to health professionals.
  6. Proactively addresses problems related to equipment, maintenance, drug availability, staffing, and patient care concerns.
  7. Identifies process improvements and safety hazards, taking appropriate actions. Participates in departmental meetings with ideas and feedback.
  8. Supports clinic, hospital, and system directives, including clinical, compliance, and customer service initiatives.
  9. Supports pharmacy leadership initiatives.

**Qualifications:**

**Minimum:**

  • Doctor of Pharmacy or Bachelor of Science in Pharmacy from an accredited institution (education verified).
  • License to practice pharmacy in the state of practice and dispense controlled substances.
  • Current knowledge of state and federal pharmacy regulations.
  • Effective communication, management, and project skills.
  • Immunization certification if applicable.

**Preferred:**

  • Experience in an integrated healthcare system.
  • Board Certification.
  • Health System Residency.

**Physical Requirements:**

  • Visual acuity for reading labels, documents, and monitors.
  • Verbal communication with providers, colleagues, patients, and visitors.
  • Manual dexterity for manipulating supplies and equipment, including frequent computer use.

**Location:** Supply Chain Center, Midvale, Utah

**Scheduled Weekly Hours:** 0 (hourly rate $59.86 - $87.98, dependent on experience)
