Job summary

Baycrest Hospital is seeking a Personal Support Worker (PSW) for the Apotex 7 Cognitive Support Unit. The role supports residents with varying cognitive and physical needs, under nursing guidance, to maintain dignity and comfort.

Ideal candidates hold a PSW certificate and have experience with elderly care, dementia, and responsive behaviors, plus strong communication and teamwork abilities.

Qualifications

• Must have a PSW certificate from a recognized college or institution.
• Recent experience with physically/cognitively impaired elderly in health care.
• Experience with mental health assessment and dementia-related care is an asset.
• Strong interpersonal, communication and problem-solving skills.
• Ability to work independently and as part of an interprofessional team.

Responsibilities

• Supports, in consultation with RN/RPN, hygiene, grooming, dressing, toileting, nutrition/hydration tracking and skin care.
• Supports client social, emotional, cognitive and cultural well-being.
• Monitors and evaluates client care needs per LTC standards and policies.
• Initiates and maintains relationships with clients, families and other care providers.

Job Summary

Apotex 7 is a Cognitive Support Unit. Residents residing on this floor are living with a wide range of cognitive and physical changes to their abilities – from mild to severe cognitive impairment and from ambulating independently, to being wheel-chair bound.

Key Responsibilities

• Supports, in consultation with the RN/ RPN, the physical and personal care needs of the client including, but not limited to: hygiene, grooming and dressing, toileting, recording nutrition and hydration intake, provision of preventative skin care
• Supports the social, emotional, cognitive and cultural well-being of the client under the guidance of the direct care team
• Uses professional judgment, monitors and evaluates client care needs and care goals regularly according to LTC standards, policies, procedures and legislation
• Initiates and maintains a relationship with the client, their family and other care providers

Qualifications

• Current P.S.W. certificate from a recognized community college or educational institution is required
• Recent placement and/or working experience with the physically and/or cognitively impaired elderly in a health care setting is required
• Experience with mental health assessment and intervention and working with responsive behaviors due to various types of dementia, delirium, and other mental health issues is preferred
• Basic computer skills – Meditech/PointClickCare, Microsoft Word and Excel is an asset
• Ability to work independently and as a part of the interdisciplinary team, and effectively manage challenging situations
• Excellent interpersonal, communication and problem-solving skills
