Job summary

Baycrest Hospital is seeking a Personal Support Worker (PSW) for a temporary part-time position in the Apotex 2 Physical Support Unit. The unit focuses on mobility, daily living activities, and emotional well-being for aging residents.

Responsibilities include assisting with hygiene, nutrition tracking, and collaborating with the RN/RPN, care team, and families to support patient goals. Prior experience with geriatric care and dementia-related needs is preferred.

Qualifications

• Current P.S.W. certificate from a recognized community college or educational institution is required.
• Recent placement and/or working experience with the physically and/or cognitively impaired elderly in a health care setting is required.
• Experience with mental health assessment and intervention and working with responsive behaviors due to various types of dementia, delirium, and other mental health issues is preferred.

Responsibilities

• Supports, in consultation with the RN/ RPN, the physical and personal care needs of the client including, but not limited to: hygiene, grooming and dressing, toileting, recording nutrition and hydration intake, provision of preventative skin care
• Supports the social, emotional, cognitive and cultural well-being of the client under the guidance of the direct care team
• Uses professional judgment, monitors and evaluates client care needs and care goals regularly according to LTC standards, policies, procedures and legislation
• Initiates and maintains a relationship with the client, their family and other care providers

Job Summary

Apotex 2 is a Physical Support Unit. This unit focuses on older persons who are living with physical conditions that result in frailty and fragility. These residents require assistance to enable them to continue to live their lives and fulfill their goals and daily activities. This unit enables staff to learn about physical conditions common with aging and the impact of these conditions on mobility, pain, activities of daily living, and emotional health.

Key Responsibilities
