Position Summary

Reporting to the Manager, Rehabilitation Services, you will work under the supervision of a regulated healthcare provider. You will provide direct, quality personal care to patients according to their care plans, aligning with BCHS’s Mission, Vision, Values, and Patient Declaration of Values.

Primary Responsibilities

  • Practice within an Interprofessional Collaborative Practice (IPC) model.
  • Follow BCHS policies and procedures.
  • Collaborate with the care team on patient care needs and outcomes.
  • Provide relevant patient information to team members as needed.
  • Assist with activities of daily living (ADLs) and observe/report changes in patient conditions.
  • Assist with personal care, transfers, ambulation, and recreational activities as per care plans.
  • Document patient care accurately following BCHS policies.

Education Requirements

  • Personal Support Worker (PSW) Certificate from a Community College.
  • Current BCLS Certification.

Mandatory Qualifications

  • Recent hospital experience preferred.
  • Ability to work effectively in a team.
  • Excellent communication skills.
  • Strong critical thinking, problem-solving, conflict management, and leadership skills.
  • Good time management and organizational skills.
  • Experience working with diverse patient populations in a hospital setting.
  • Excellent attendance and reliability; ability to work all shifts.

Mandatory Competencies

  • Commitment to safety for patients, workers, and the workplace, with good stewardship in identifying and mitigating unsafe acts or conditions.

Preferred Competencies

  • Reliable with excellent attendance and flexibility to work all shifts.
