Job summary

Saint Elizabeth in Collingwood, ON is recruiting Registered Nurses to join our team. You will perform comprehensive client assessments, develop and implement care plans, and collaborate with a multidisciplinary team to deliver high-quality, evidence-based home care.

Qualifications

  • Current registration with the College of Nurses of Ontario is required.
  • CPR certificate is required.
  • A valid driver’s license and vehicle, travel required.
  • Willingness to attain Saint Elizabeth certifications.
  • Availability to work days, evenings, nights and every other weekend.
  • This is a physically demanding role with lifting up to approx. 55 pounds.

Responsibilities

  • Complete comprehensive client assessments, develop and evaluate care plans based on best practices and client needs.
  • When designated as primary provider, lead the care for a client caseload with the care team.
  • Support clients through transitions by connecting with community support services.

Responsibilities:
  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
  • When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
Requirements:
  • Current registration with the College of Nurses of Ontario
  • Canadian Nurses Association (CNA) specialty certification an asset
  • Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
  • A current CPR certificate is required
  • A valid driver’s license and vehicle (travel required)
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required Saint Elizabeth certifications.
  • Availability to workdays, evenings, nights and every other weekend.
  • This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
  • Must be available to work from 12 hours Monday- Sunday 9a-9pm at Collingwood Clinic (186 Erie Street- Suite 203).
