Qualifications

  • Registered Nurse or Registered Practical Nurse with pediatric and palliative care experience.
  • Current CPR certification and valid driver's license required.

Responsibilities

We're currently hiring Full-Time and Part-Time Overnight Shift Nurses (RN and RPN) to support our pediatric and palliative programs

We are seeking dynamic, passionate Nurses who want to be part of transformational change in home and community care by providing care in a truly client-centered approach.

SE Health is proud to partner with CHEO to provide care and support to children and families who need home and community care services. The Pediatric Nurse is an important member of the child's care team and will make a meaningful difference in their life.

SE Health also provides palliative care services supporting individuals facing life-threatening illnesses and their families. Our team of nurses collaborates with other healthcare professionals to deliver comprehensive care addressing physiological, psychological, socio-cultural, and spiritual needs. Our community-based palliative care team aims to help people live actively until death and support families during illness and bereavement.

Responsibilities:
  1. Complete comprehensive client assessments, develop care plans, and evaluate care based on best evidence and client needs.
  2. When designated as a primary provider, lead care for a client caseload, collaborating with the care team.
  3. Support clients through care transitions by connecting them with community support services.

Minimum Requirements:
  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification is an asset.
  • Passion for community care and evidence-informed practice.
  • Pediatric competencies preferred.
  • Hospice Palliative care experience and ongoing education preferred.
  • Current CPR certification.
  • Valid driver’s license and vehicle (travel required).
  • Updated immunization records, criminal background check, and Vulnerable Sector check upon hire.
  • Willingness to attain SE Health certifications.
  • This position is physically demanding; requires dexterity, sensory ability, lifting up to 55 pounds, and mobility tasks.
