Job summary

Sobeys Inc. is recruiting for a Pharmacist to join our national pharmacy team. You will provide patient-centered care, counsel on therapies, and lead documentation and follow-ups to support safe, effective outcomes across Sobeys banners in Canada.

Licensed pharmacists in good standing are essential, with a focus on quality service, regulatory compliance, and collaboration with pharmacy technicians and colleagues to deliver optimal patient care.

Qualifications

• Licensed Pharmacist in good standing in the province of responsibility.
• Bachelor of Science in Pharmacy from a recognized institution.

Responsibilities

• Demonstrate warmth, empathy, and compassion while acting in the patient’s best interest to achieve excellence in patient-centered care, customer service, workflow efficiencies, and professional clinical services (e.g., disease state management, medication reviews, adherence, health risk assessments, injections/vaccines).
• Provide patient counseling and education for new therapies to patients and caregivers, tailoring the approach to the needs and concerns of the patient (e.g., literacy, hearing loss, privacy).
• Initiate care plans, follow up, and complete proper documentation.
• Participate in professional programs and collaborate with colleagues to provide excellence in health and wellness services to all patients.
• Actively engage in all areas of Fusion to ensure patients experience seamless care and a consistent pharmacy experience across locations.
