Bayshore HealthCare in Ontario is seeking a Client Scheduling Coordinator to act as the vital link between patients, families and healthcare providers. You will manage referrals, schedule care, and ensure accurate EMR documentation, while delivering excellent customer service.

Qualifications

• Minimum two years of customer service experience, preferably in healthcare.
• Excellent written and verbal English communication.
• Ability to prioritize and manage multiple tasks in a fast-paced environment.

Responsibilities

• Communicate with patients, families, staff and funder organizations.
• Process funder referrals and updates in the EMR (Procura).
• Review service needs with Supervisors, field staff and patients.
• Schedule care providers based on client needs, geography and availability.
• Coordinate caregiver assignments and service delivery.
• Notify patients and staff of initial and ongoing schedules.
• Utilize scheduling software to optimize staff availability.
• Document records accurately in the EMR.
• Resolve patient concerns promptly with escalation as needed.
