Detailed Overview

In accordance with the BC College of Nurses and Midwives (BCCNM) standards and Fraser Health’s values, the Patient Care Coordinator provides clinical supervision, guidance, and support within the assigned units, overseeing daily patient flow and participating in research, policy, quality improvement, and risk management activities.

Responsibilities

• Manage patient admissions, discharges, and referrals, ensuring optimal patient flow and care planning in collaboration with the clinical team.
• Supervise nursing staff and coordinate daily clinical operations, including staffing, scheduling, and quality monitoring.
• Promote interprofessional collaboration, mentor staff, and support clinical practice excellence.
• Lead or support care conferences and rounds, disseminate clinical information, and support staff education and development.
• Address patient and family concerns, participate in complaint investigations, and collaborate on quality improvement initiatives.
• Assist in staff recruitment, performance evaluation, budgeting, and resource management.
• Participate in research, committee work, and policy development.
• Perform other related duties as assigned.

Qualifications

Education and Experience

Current RN registration with BCCNM, with at least 3 years of recent clinical experience in the designated area, including 1 year of supervisory experience, or equivalent education and training. Postgraduate coursework or certification is preferred.
