Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, working collaboratively with the inter-professional care team, the Client Care Coordinator provides clinical supervision, guidance, and support to nursing staff, clinical team members, and support staff within the Urgent Primary Care Centre (UPCC). The UPCC provides timely urgent care and continuous longitudinal primary care to unattached vulnerable clients in the region. Responsible for daily operations, patient flow, and supporting research, policy, quality improvement, and risk management activities. Duties are performed through a trauma-informed and evidence-informed approach, supporting clients and families in navigating the healthcare system and supporting regional activities.

Responsibilities
  • Lead, facilitate, and coordinate daily intake and patient flow using established criteria in collaboration with the clinical team.
  • Supervise nursing, allied, and support staff; coordinate work, monitor quality and safety, assign schedules, and ensure staffing levels meet service requirements.
  • Promote inter-professional collaboration and team building through meetings and discussions, supporting quality patient care.
  • Mentor and serve as a resource to staff, role model best practices, and assess clinical skills for performance improvement.
  • Identify staff learning needs, develop educational objectives, and evaluate learning outcomes through various educational activities.
  • Participate in or lead investigations of patient/family complaints, follow-up, and corrective actions, collaborating with Quality Improvement.
  • Engage in quality improvement and risk management activities, including safety audits and practice evaluations.
  • Assist the Manager with staff recruitment, performance reviews, budget input, and equipment needs.
  • Encourage staff involvement in research, evaluation, and special projects.
  • Participate in professional and operational committees at various levels.
  • Contribute to the development and evaluation of clinical decision support tools and policies.
  • Perform other related duties as assigned.
Qualifications
Education and Experience

Current registration as a Registered Nurse with BCCNM, with three years of recent clinical experience in acute or urgent care, including pediatric and adult care, plus one year of supervisory experience, or an equivalent combination. Post-graduate coursework or certification in relevant clinical areas is preferred.
