Acclaim Health in Oakville is seeking a compassionate Part Time Registered Nurse specializing in Palliative Care to provide in-home, patient-centered care across the Halton community.

Qualifications

• Current registration with the CNO
• Minimum 1–2 years in a community setting, preferably palliative care
• CPR certificate required
• Valid driver’s license and access to a vehicle

Responsibilities

• Provide comprehensive nursing care to palliative patients in their homes in the Halton area
• Organize the day and set priorities based on continuity of care
• Develops and modifies the plan of care to achieve outcomes
• Assess the health status of the client
• Administer IV therapy and line maintenance
• Administer medications topically, via tubes, or by injections
• Manage wound care for various wounds
• Ensure laptop synchronization to update client records
• Attend team/POD meetings as required
