Job summary

Spectrum Health Care is seeking part-time/casual Registered Practical Nurses to join a dedicated team delivering integrated palliative care. You will work with a multidisciplinary team to provide holistic, patient-centered care across their palliative journey.

Responsibilities include symptom management, care coordination, and education for patients and families. Valid Ontario RN status, police check, CPR/First Aid, and ability to travel are required.

Qualifications

• Currently registered and in good standing with the College of Nurses of Ontario.
• Minimum of two years nursing experience; two years of palliative care preferred.
• Experience with palliative care patients.
• Police check suitable for working with vulnerable sector.
• Valid Ontario driver's license and reliable vehicle; travel required.
• Current CPR and First Aid certificates.
• Competence in wound care, IV therapy, chronic disease management, and palliative/pain management.

Responsibilities

• Work under the supervision of the Integrated Care Program Supervisor.
• Collaborate with the interdisciplinary team to create personalized care plans based on patient needs.
• Participate in clinical huddles and case conferences with physicians and allied health staff.
• Practice in accordance with regulatory standards and Spectrum Health Care policies.
• Travel within the assigned region between patients' residences or facilities.
