Job summary

Qualifications

• Registered nurse with current Ontario registration.
• Palliative care experience including advance care planning and end-of-life care.
• Experience linking with community health agencies and coordinating care.
• Two years of related nursing experience is preferred.
• COVID-19 vaccination policy applies to all staff.

Responsibilities

• Perform comprehensive patient assessments and determine care needs.
• Assess palliative care eligibility and hospice admission.
• Develop individualized care plans reflecting patient priorities.
• Coordinate referrals to hospice and palliative services.
• Support MAiD coordination where required.
• Maintain open communication with care providers and patients.
