Job summary

Connective is a dynamic non-profit organization providing innovative programming in social services. This role delivers casework and clinical support for residents, aiming to enhance quality of life and ensure safe operation of the facility.

The incumbent will work under the Program Manager, participate in team meetings, and collaborate with outside professionals to implement care plans and therapeutic strategies.

Qualifications

• Degree or Diploma in mental health field required.
• Experience with crisis situations preferred.
• Knowledge of Psycho-Social Rehabilitation applications.

Responsibilities

• Interviews clients to prepare care plans with confidentiality.
• Acts as primary or key worker with assigned resident(s).
• Connects clients with health and psychiatric services.
• Dispenses medications and monitors adherence as required.
• Supports residents integration into the community.
