Job summary

Jobtailor is hiring a Home Health Nurse to coordinate at‑home patient care across Ontario. You will assess needs, plan and monitor care, and collaborate with interdisciplinary teams to ensure high-quality outcomes for diverse clients.

The role requires RN or RPN, valid First Aid/CPR, and a clean Ontario license. Expect a bi‑weekly weekend rotation, strong documentation, and ongoing professional growth.

Qualifications

• Registered with the College of Nurses of Ontario in good standing.
• RPN or RN diploma/degree from a recognized college or university.
• Valid First Aid or CPR Certification.
• Ontario Driver's Licence (G) and reliable vehicle for travel.
• Morning and day availability; bi‑weekly weekend rotation.
• Strong written and verbal communication and documentation skills.
• Reliable attendance, punctuality and availability.
• Commitment to lifelong learning and professional growth.
• Ability to apply critical thinking to client care.
• Membership in RNAO/WeRPN preferred; knowledge assets welcomed.

Responsibilities

• Provide and coordinate home health nursing services to diverse individuals and groups.
• Assess, monitor, and plan care; coordinate care accordingly.
• Evaluate care plan interventions through ongoing reassessment.
• Ensure quality patient care by addressing factors affecting practice and delivery.
• Demonstrate professionalism, leadership, judgment, and accountability.
• Support safe client care via quality improvement activities.
• Inform the nursing supervisor of client changes or concerns.
• Submit appropriate documentation to the case coordinator.
• Maintain documentation per College of Nurses of Ontario standards.
• Collaborate within ISDS to strengthen integrated care and client experience.
