Job summary

Ontario Health atHome seeks two Float Hospital Care Coordinators to provide coverage for all patient services programs across the Toronto Central region. Travel to hospital sites may be required; the role focuses on delivering connected, patient-centered care with a collaborative team of health professionals.

Your responsibilities include assessing care needs, coordinating with families and service providers, and ensuring documentation is complete and accurate while supporting growth and balance

Qualifications

• Membership in good standing with applicable regulatory body.
• 1+ years of recent experience in community health.
• Knowledge of the health care delivery system and community resources.
• Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
• Effective time management and ability to work independently in a busy multidisciplinary environment.
• Valid driver’s license and access to a vehicle.
• Proficient in Windows.

Ontario Health atHome is seeking two qualified candidates to provide float coverage for all patient services programs. The position may involve travel to all hospital sites throughout the geographical region of Ontario Health atHome Toronto Central. Please note, you may be required at multiple hospital locations.

What will you do?

• In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans
• Link patients with service providers
• Coordinate and monitor care plan delivery
• Establish a helping relationship with patients and their families
• Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected

What must you have?

• Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • College of Dietitians of Ontario
  • College of Audiologists and Speech Language Pathologists of Ontario
• 1+ years of recent experience in community health or a related field
• Knowledge of the health care delivery system and community resources
• Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
• Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
• Established ability to accurately complete required documentation, reports and forms
• A valid driver’s license and access to a reliable vehicle
• Proficient in a Windows environment
• We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

• Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
