LOFT Community Services is hiring a Personal Support Worker for the Back to Home Program in Orillia, ON. This overnight position supports seniors 55+ with high care needs across multiple sites, delivering personal care, health monitoring, and social engagement.

You will work within a multidisciplinary team, coordinating with health professionals and documenting care plans. A PSW certificate and First Aid/CPR are required; driver’s license is preferred.

Qualifications

• Post-secondary education in Human Services or a related field.
• Experience supporting seniors and vulnerable populations.
• Current First Aid and CPR certification.
• Valid Ontario Driver’s License is preferred.

Responsabilités

• Provide compassionate client care and support for physical and mental health needs.
• Assist with activities of daily living and maintain a safe living environment.
• Engage clients and promote dignity, independence, and participation.
• Support behavioral and emotional wellbeing and crisis intervention as needed.
• Follow safety procedures, respond to emergencies, and report incidents.
• Support medication protocols and coordinate with healthcare providers.
• Maintain accurate care records and participate in care planning.
• Assist with program activities and day-to-day operations as required.
• Work rotating shifts and provide coverage as needed.
• Additional duties may be assigned by the supervisor.
